Tailored Support For Young People
At Midway Transitional Solutions, our outreach services provide tailored support for young people 16 and over in the community, ensuring they receive guidance and care even outside the home. We work closely with social workers, families, and other professionals to address a range of needs, including social, emotional, and behavioural challenges.
We Aim To Empower Young People
Our outreach team supports young people to:
- Manage and understand challenging behaviours.
- Engage positively with home, school, and other authority figures.
- Navigate the Youth Justice System and make safer life choices.
- Build resilience after previous failed placements or difficult experiences.
- Access counselling, therapy, or specialist services for issues such as substance misuse, self-harm, or experiences of abuse.
Through our outreach work, we aim to empower young people, help them develop essential life skills, and support their successful integration into society—all while providing continuity of care and a positive, consistent presence in their lives.
