At Midway Transitional Solutions Limited we recognise the importance of choice, dignity and respect for all children and young people in our care and the need for them to be valued members of society. The core value of all decisions, interventions and interactions will be the safety, welfare and positive regard for each child and young person.
For All Services We Ensure
A Personal Plan will be developed before the child or young person begins to receive care and support, following admission to the home, this will be fully updated within the first 7 days. In the case of emergency placements, the personal plan will be completed within 24 hours and highlights what services the home will provide.
- A health plan that is regularly updated and is overseen by the manager
- Appropriate therapeutic interventions if identified from suitably qualified personnel
- The preparation and provision of an integrated care package tailored to individual needs and to include full risk assessments
- The development of pathway plans for children over 16 focusing on life skills, further education and career aspirations
- Support in maintaining mainstream school or college attendance
- Preparation of future placements, prior to discharge from the home
- Encouragement and support in developing their own interests and activities within the local community
